There used to be no professional pay-TV systems for medium and small operators.  Why? 
A main reason is the CAS market dynamics:  The larger the operation, the more robust the pay-TV system must be against piracy.  Costs rise with larger responsibility, and CAS prices therefore don’t fall (much) with volume.

Also, with their security reputation being very important, CAS vendors impose restrictions on the integration and use of their CAS. This also add costs.
With branded CAS being too complex or expensive, less serious solutions have mostly been the options offered small/medium operators.
